Instructions: Read the included NTSB report, which is packaged in the course. Identify no more than two human factor issues that you feel were significant elements in the error chain leading to the terminal event. Defend your position by accurately and succinctly showing the “cause and effect” of those dynamics. It is important to consider the levels within the Human Factor Classification System (HFACS) as you assess the report.

Rubric: The grading rubric is based upon APA writing style, critical thought, a detailed analysis of information and assessment of the “cause and effect” of Human Factors in this aviation accident. Be sure to provide an introduction and conclusion in your report and be sure to cite additional references beyond the NTSB report, if needed.
