The SPP is a proposal for a public health program operating on a 3-year grant with an annual budget of $300,000. Based upon these parameters, the SPP includes details of how you would develop this program, including the details of the budget and timeline. Submit your proposal for your Scholar-Practitioner Project. Your proposal should include the following: 20pages

Introduction (descriptive narrative): Overview

 An overview of the public health issue and program

 Justification of the need for the program, based upon the results of your community health assessment,

 A summary of the theory you chose and how you integrated it into the MAP-IT Model in developing the public health program. (Week 2)

 Goals and objectives that are specific, measurable, achievable, relevant, and timely (SMART). (Week 3)

Body:

 A Logic Model and justification (Week 4)

 A budget and justification (Week 4) o Include a detailed budget of how the $300,000 would be spent and why

 The timeline and justification (Week 4) o Include a detailed timeline of what would occur during the 3-year grant period  A social marketing plan including rationale and ethical considerations (Week 6)

 An implementation plan including setting and stakeholders (Weeks 7 and 8)

 An evaluation plan (Week 9)
